1. Download & complete a copy of our Registration Form 2011 located on the MKYBL Forms page.
2. (1) copy of your child's Birth Certificate.
3. (1) copy of your child's Medical Insurance Card.
4. Payment by cash or check for Registration Fee or Total Cost. Please make checks payable to MKYBL.
There will be a service charge of $15.00 for all returned check & and a non-refundable $40.00 processing fee. MKYBL will establish a cut-off limit on the total number of rostered players for each division. Completed registration form & paid registration fee or total cost will constitute a rostered player. All others may be placed on a wait list if limits are reached. No refunds will be given afer January 15, 2011.
